This form applies to the management of recreation facilities located on university campuses. It should be used as a risk identification tool as part of a regular risk assessment process. This checklist can be used by all Facility Managers, Staff and Instructors to maximise the likelihood of positive outcomes for university recreational programs.

This checklist is generic and may not be suitable for all university recreation centres. It is recommended that each facility utilising this checklist, employ modifications to ensure it is appropriate for their individual circumstances and to better identify and mitigate the risks inherent in their facility.
