This form applies to all off-campus activities which occur at a distance greater than 100 kilometres from the University. Examples of such travel include: fieldwork, conferences, preceptorships, research, international exchange studies, work experience, University sponsored volunteering and business related travel.

This checklist should be employed prior to undertaking travel and in conjunction with University travel policies and procedures. The University recognises the significance of long-distance travel to teaching, learning and research; however, such travel offers unique risk exposures. The University and its community have obligations under the relevant OHS statutes and it is the responsibly of all members of the community to ensure that relevant polices and processes regarding safety and travel are followed.
